Tailored Care Management (TCM)

Tailored Care Management at B&D Integrated Health East helps individuals manage physical health, behavioral health, and everyday care needs through coordinated, person-centered support. Each client is connected with a dedicated Care Manager who helps identify needs, create a care plan, and stay focused on wellness goals.
